Required Tranportation Info:

  1. Patient Location?
  2. Level of service requested and the type of equipment needed?
  3. Caller name and call back number?
  4. Requested time of pick-up?
  5. Patient destination?
  6. Patient’s Last Name, First Name, Age?
  7. Patient’s insurance coverage
    a.  Medicare – Complete and /or fax PCS form
    b.  Medicaid – Contact Broker and request Medshore Ambulance Service
    c.  Private Insurance – Carrier, policy number, billing address
  8. Medshore will confirm call prior to transport
